What to bring to your appointment
- Your valid GP or Specialist referral
- Medicare card, Private Health Care Fund details/card, DVA and/or Pensioners card
- X-rays, CT scans and/or MRI scans relevant
- Blood and urine results (or any other results that may be relevant)
- List of your current medications
- List of any allergies
- Previous reports about relevant operations
- Interpreter (if required): Please inform our staff if you require an interpreter so one can be arranged for you
- Payment: A fee is required when attending the consultation with your doctor which may be explained when you make your appointment over the phone. You may be able to receive a rebate on part of this fee from Medicare. Consultation fees will need to be settled on the day of your appointment
- List of questions regarding your operation, condition or treatment plan
