What to bring to your appointment

  1. Your valid GP or Specialist referral 
  2. Medicare card, Private Health Care Fund details/card, DVA and/or Pensioners card 
  3. X-rays, CT scans and/or MRI scans relevant 
  4. Blood and urine results (or any other results that may be relevant)
  5. List of your current medications
  6. List of any allergies
  7. Previous reports about relevant operations
  8. Interpreter (if required): Please inform our staff if you require an interpreter so one can be arranged for you
  9. Payment: A fee is required when attending the consultation with your doctor which may be explained when you make your appointment over the phone. You may be able to receive a rebate on part of this fee from Medicare. Consultation fees will need to be settled on the day of your appointment
  10. List of questions regarding your operation, condition or treatment plan
